The first is that, when the rubber hits the road, insurance companies are businesses. They have shareholders and governing boards to please, and that means trying to cut costs and increase profits. One way to do that is to pay out fewer claims, so they will often deny claims just to see who will fight back. The harsh reality is that for companies this size, it can be cheaper to pay legal fees than to pay claims.Why doesn't the insurance company believe you?

The second reason is that they're really are fraudulent claims out there. Claim rejection is, unfortunately, a natural reaction over time to people trying to game the system.
